What is the length of placement?

Length of placement varies depending on individual needs. Because Prader-Willi syndrome (PWS) requires lifelong support, placements are typically long-term. Our community-based residential settings are less restrictive and designed to provide a permanent home for residents—offering safety, independence, structure, and a deep sense of belonging.

What types of programs and services do you offer?

Our Health & Wellness, Residential, Respite, Day R.I.S.E., and Consultation Services are tailored to support adults with PWS. From individualized dietary and wellness plans to 24-hour residential supervision, specialized therapeutic support, structured day programming, and expert consultation—we provide holistic, person-centered care focused on long-term success and community connection.

What vocational and day services are provided?

Residents have access to:

  • Work and vocational services through community partners.
  • On-site day programming through our Day R.I.S.E. program, which includes structured activities, personal development, and community collaboration.

What is Day R.I.S.E.?

Day R.I.S.E. is a weekday program that offers structured social, learning, and recreational opportunities. Participants engage in four unique activities each day, tailored to support personal growth. The program runs from 8:00 AM–2:30 PM, Monday through Friday, and can be combined with other work, volunteer, or educational activities.

What is the placement process for PWH?

Placements are typically initiated by families or referring agencies. Our admissions team requests relevant background information and then coordinates a tour and face-to-face meeting (virtual options available). If we can meet the individual’s needs, a pre-admission packet is shared, contracts are finalized, and a move-in date is established.

What role does family play in treatment?

Family involvement is essential. Families are considered part of the treatment team and are encouraged to participate in planning, communication, and visits. We believe in supporting both the individual and their family through collaborative, consistent care.

What medical services are offered?

Residents are supported by a trained Health Services Team and direct care staff trained in:

  • Medication administration
  • RN delegated tasks
  • First aid and CPR
  • Medical appointment coordination and documentation We work closely with external providers and support residents in advocating for their health needs while helping families navigate insurance, specialty care, and communication with prescribers.

How is diet managed?

All residents receive individualized dietary plans developed by a registered dietitian. Menus are reviewed regularly and adapted as needed. Due to hyperphagia, food environments are structured with security features like locked storage and staff-supervised access. Residents participate in meal planning in a safe and structured manner.

What exercise opportunities are provided?

Residents are offered daily opportunities to be active through:

  • Walking, biking, and nature hikes
  • Exercise groups and videos
  • Fitness equipment
  • Games and dancing
  • Special Olympics sports including basketball, swimming, bowling, bocce, and track & field

What activities are planned for residents?

Each home creates a monthly activity calendar filled with:

  • Community outings (movies, libraries, shopping, dining)
  • In-house activities (crafts, social events, game nights)
  • Special events and seasonal celebrations (haunted houses, dances, Chinese New Year) Activities are based on resident interests and chosen collaboratively.

Where are the homes located?

Our homes are located in and around beautiful Oconomowoc, Wisconsin—a peaceful, safe, and welcoming community offering access to local parks, trails, medical services, and a variety of recreation opportunities.
